Nevada Updates Provider Credentialing Requirements with New Uniform Forms

NV| Bulletin 25-004, issued by the Nevada Division of Insurance, reminds insurers and managed care entities to use the recently updated Uniform Credentialing Form NDOI-901 and its new associated forms (Addendum NDOI-901A and Facility Credentialing Application NDOI-901B) as prescribed when obtaining health care provider credentials. These consolidated forms are required for applicants seeking participation with health plans, hospital staff membership, or clinical privileges, and aim to standardize credentialing submissions statewide.
