Maine Issues 2027 Network Adequacy Guidance: Carriers Must Meet Strict Time and Distance Standards

ME| Maine Bureau of Insurance Bulletin 492 (superseding Bulletin 487) sets network adequacy guidance for 2027 Qualified Health Plans, requiring carriers to continue using the 2026 CMS-based time and distance standards so that at least 90 percent of enrollees in each county can access specified provider and facility types within defined travel times, with limited county- and specialty-specific modifications where federal baselines are not achievable. The Bulletin confirms use of CMS’s geographic distance methodology, retains Rule 850’s requirement of at least one full-time-equivalent primary care provider per 2,000 enrollees (with primary care defined to include general and internal medicine, family practice, and pediatrics), and provides detailed county-by-county mileage and minute thresholds for numerous individual and facility specialties in Appendix 1, including alternative standards highlighted for certain combinations. Carriers must submit network adequacy templates in SERFF (one network file per network, excluding separate dental templates), may request exceptions using a simplified Appendix 2 form explaining any failure to meet standards, and must follow CMS’s 2027 submission schedule: initial data due June 5, 2026, optional second submission July 15, 2026, and final submission August 14, 2026, after which the Bureau and its vendor Quest Analytics will review adequacy and notify carriers of deficiencies.
