- Life and health insurers: TOI = L08 Life-Other or ML02-Multi-Line Other. Auto insurers: TOI = 19.0 Personal Auto.
- One filing per insurer; attestation uploaded under the Supporting Documentation Tab; Form Schedule Tab left blank.
